CMS Posts Eligibility Tool for CPC Initiative

CMS has posted a  Comprehensive Primary Care Initiative practice application eligibility tool on its website to help primary care practices in the Dayton and Cincinnati area determine whether they will be eligble to apply for the Initiative once Memoranda of Understanding have been completed by participating payers in the Cincinnati-Dayton market.

Primary care practices may begin to use the application eligibility tool now, and it will be held open on the CMS Innovation website for the entire practice application phase.  Practices should note that the full list of eligibility criteria have not yet been released.  The practice application eligibility tool is not  the practice application for the Initiative.  However, completion of the practice application eligibility tool, which includes leaving a point of contact email address for the practice, is a prerequisite for receiving the URL of the online application itself once it becomes available. 

Practices must complete and submit the CPC initiative application in order to be considered for selection in the CPC initiative.  CMS has begun outreach to primary care practitioner groups, and will be disseminating the link to the practice application eligibility tool to contacts.
