CMS Accepting Care Coordination Initiative Applications

Dayton-Cincinnati Practices Must Apply by July 20th

CMS has posted a  practice application eligibility tool on its website to help primary care practices in the Dayton and Cincinnati area determine whether they will be eligble to apply for the Comprehensive Primary Care Initiative. Applications from primary care practices are currently being accepted  through July 20th and the primary care practice solicitation (PDF) describes the initiative eligibility and selection.

CMS will hold a webinar June 19, 2012 from 10:30 am until noon, EDT to introduce the Comprehensive Primary Care Initiative (CPCI) to the primary care provider community in the seven national pilot areas. The webinar will focus on the CPCI's service delivery model, including the five Comprehensive Primary Care functions, and the milestones participating practices will need to achieve in the first year. The process for applying will also be covered, including information about the application itself. The CPCI is a multi-payer initiative fostering collaboration between public and private health care payers to strengthen primary care. Medicare will work with commercial and State health insurance plans and offer bonus payments to primary care doctors who better coordinate care for their patients.

Counties within this region include  Adams, Brown, Butler, Champaign, Clark, Clermont, Clinton, Greene, Hamilton, Highland, Miami, Montgomery, Preble, Warren and the Kentucky counties of Boone, Campbell, Grant, Kenton

Primary care practices that choose to participate in this initiative will be given resources to better coordinate primary care for their Medicare patients. Participating doctor practices will initially be paid an average of $20 per patient per month to better coordinate patient care.

The first step of the application is the Application Pre-Screening tool if you are a provider interested in applying. Upon completing the tool, applicants meeting the minimum level eligibility requirements will receive an email with further details about the upcoming application and how to apply to the program. If your primary care practice has multiple sites, each physical site that is located in the selected market is eligible to apply and must apply separately.

All applications must be submitted by 5:00 pm ET on July 20, 2012.

Approximately 75 primary care practices in each designated market will be selected to participate in the CPC initiative. Nine commercial insurers and Ohio Medicaid were approved to participate in the region’s new pilot primary care initiative. Aetna, Amerigroup, Anthem Blue Cross Blue Shield of Ohio, Humana, UnitedHealthcare, CareSource, HealthSpan, Medical Mutual, Centene Corp. and Ohio Medicaid are the payers that signed agreements, according to The Health Improvement Collaborative of Greater Cincinnati.
