The American College of Osteopathic Family Physicians is now accepting applications for the 2013 ACOFP Fellows Award. The award is designed to recognize individual members who have contributed outstandingly through teaching, authorship, research, or professional leadership, either state or national, to the stated purposes of the ACOFP. Applicants must:
Ohio applicants must be nominated by a current Fellow. Contact the Ohio ACOFP office or any of the following ACOFP Board members for assistance: Robert L. Hunter, DO, FACOEP, FACOFP; Geraldine N. Urse, DO, FACOFP; or Christopher J. Loyke, DO, FACOFP.
Applications must be submitted through the sponsor and accompanied by (1) a letter of endorsement from the sponsor and the ACOFP state society (or a second fellow from Ohio); (2) a completed, unpublished scientific paper pertinent to family practice that should be submitted to the sponsor one month in advance for review and editing, and for a written appraisal, which will be made part of the applicant;’s file; and (3) a check for $195. Unsuccessful candidates will have a portion of the fee returned. If the application is approved, an additional fee of $375 is required for cap and gown rental, ceremony expenses and a plaque.
