Full-Time, Classified, Hourly, Non-Exempt. 37.50 hours per week, Monday – Friday, with occasional evenings and weekends. The pay range for this position is $43.27 - $48.28 per hour, based on experience. Partial funding for this position is provided by the Preconception Health & Wellness Program grant.

 

This is a primarily clinic-based position; with occasional telehealth and/or community outreach. Medical care will be monitored by peer review and continuous quality improvement.

 

Provide primary and specialty care services and perform clinical procedures for the Health Center. Conduct comprehensive physical examination and obtain medical history; diagnose and treat acute and chronic illnesses. Administer oral medication and subcutaneous and intramuscular injections, immunizations, and medications. Prescribe medications and treatments; order and interpret diagnostic tests. Formulate care plans and manage patient care. Provide education and counseling on health maintenance and disease prevention. Perform Clinical Laboratory Improvement Amendments (CLIA) waived testing. Prepare and/or provide patient follow-up and respond to patient calls and messages. Complete and maintain documentation in the electronic medical records. Assist with completing and documenting grant deliverables. Complete prep work and assemble supplies for off-site and mobile clinics. Conduct investigations, case management, and surveillance as required for all reported communicable diseases. Follow required guidelines for communicable disease reporting. Work with the Epidemiologists on surveillance, quality assurance, and outbreaks. Provide reports when directed. Develop and maintain partnerships with health professionals. Attend related training and educational opportunities on communicable disease program management. Assist with the inventory, ordering, and security of clinic supplies and pharmaceuticals; and performing refrigerator and temperature checks. Conduct presentations relating to important health care issues and preventative measures.

 

Must be detail oriented, organized, possess strong oral and written communication skills and listening skills, excellent time management skills, strong customer service skills, ability to multitask, and work well as part of a team. Ability to exhibit a high degree of courtesy, tact, and poise when interacting with claimants, families, and other healthcare professionals essential. Proficiency in Microsoft Office and a basic knowledge of electronic medical record systems necessary. A Master’s Degree in Nursing; an unrestricted Ohio Registered Nursing license; a current Certificate of Authorization with national board certification as a Family Nurse Practitioner; and CPR certified for adults, children, and infants required. Must be able to maintain all required licenses and certifications and meet Ohio Board of Nursing and State requirements. A valid Ohio Driver’s License, valid auto liability insurance, the ability to maintain auto liability insurance, and the ability to maintain insurability under the County’s vehicle insurance policy essential. Must be able to lift, carry, push, and move objects, equipment, files and/or packages weighing up to 25 unassisted and up to 50 pounds with assistance necessary. Previous experience working in a public health

environment, administering and inventorying immunizations, and program coordination are beneficial. At least one (1) year of Nurse Practitioner experience is preferred.
