I never expected my path in medicine to look quite like this. I started as a combat medic—young, hungry, and trying to absorb everything I could. Over the years, I became a civilian paramedic, working long shifts in prehospital care while still drilling with my Army Reserve unit. Whether I was managing an airway in the back of an ambulance or leading a trauma lane during a field exercise, I was, and am, constantly balancing two worlds. But those two worlds had more in common than I realized: the unpredictability, the need for calm under pressure, and the ability to lead in the chaos.

Lt Col (ret) Torree McGowan, MD, FACEP

GSACEP commissioned this interview with retired Lt. Gen (Dr.) Paul K. Carlton, Jr. as part of its History of Military Emergency Medicine project. At 69, Dr. Carlton continues to work at Texas A & M University in Bryan, TX. During his distinguished career in the United States Air Force, Dr. Carlton, Jr. served as the 17th Surgeon General of the Air Force (1999-2002). Prior to that assignment, he was the Commander and Director of the Air Force Medical Operations Agency (1999-2002) at Bolling AFB, Washington, D.C. His story and contributions to military emergency medicine starts in 1972 when, at 25, he co-authored a legislative bill and completed his senior project to establish an emergency medical services system while attending medical school at the University of Colorado.

Point-of-care ultrasound (POCUS) has led to significant changes to patient care and outcomes in hospital environments and has since spread to pre-hospital and austere environments. Over the past several decades, increased portability of ultrasound has allowed for its use and demonstrated efficacy in numerous resource-limited settings, including low and middle-income countries, the Iraq and Afghanistan wars, and natural disasters. The introduction of handheld devices has further increased the portability and ease of use of POCUS with reliable diagnostic accuracy. The intent of this document is to provide helpful advice to providers who will or are operating in austere environments in training others in POCUS.

Throughout the current Russo-Ukrainian War, Russian forces have consistently targeted Ukrainian medical infrastructure. From February 2022 to January 2025 alone, the World Health Organization recorded at least 2,215 attacks on Ukrainian healthcare, including a significant number of attacks on maternity wards and pediatric centers. Ukrainian forces have adapted by embracing hardened, underground facilities, offering a glimpse of the facilities that American forces may require during a future large-scale combat operation (LSCO).
