Clinical Dietitian
The Renaissance Retirement Campus
Clinical Dietitian (full-time or part-time)
This position will serve at Eliza Jennings Home, located in Cleveland and The Renaissance Retirement Campus, located in Olmsted Township.
Summary:
Oversee the management of nutritional therapy and dietary needs for residents in the philosophy of person-centered care and in compliance with medical directives.
Duties include:
- Serve as a member of the interdisciplinary team, participating in the development and maintenance of plan of cares, policies, and procedures.
- Provide time specific MDS-3.0 assessments. As necessary, meet with interdisciplinary care team to discuss the comprehensiveness of assessments and develop the resident plan of care.
- Meet residents and their representatives as required to discuss nutritional status and future goals.
- Develop individual care studies of significant unintended weight loss. Track tube feedings, supplement use, diet order distribution, adaptive equipment, and pressure ulcer status in accordance with person-centered care and quality assurance.
- Complete monthly weight report and document all significant weight changes with assessments and interventions accordingly. Share information with appropriate staff. Track pressure ulcer status.
- Provide nutrition counseling and therapy for residents for specialized diet orders, unintentional weight loss, altered skin integrity and other as consulted.
- Provide staff education as requested.
- Initiate and monitor the use of selective menus for residents. Adjust menus for resident preferences, therapeutic needs and nutrition needs. Use nutritionally equivalent substitutes when required.
Requirements:
- 2 years' experience as a dietician and in medical nutrition therapy in the geriatric population, particularly in long term care.
- Licensed and Registered with the American Dietetic Association’s Commission on Dietetic Registration in the State of Ohio.
- In-depth understanding of nutrition and Medical Nutrition Therapy in geriatric population.
