Split-Shared and Incident-To visits have seen changes in their identity in the past 5 years as both CPT and CMS/Medicare have better defined them. No longer is a hospital note that states the physician agrees with the Non-Physician Practitioner’s (NPP) plan sufficient.

But what is?

How can a physician efficiently document to show their substantive involvement in the patient’s care?

What information is needed in a plan of care in the office setting to allow for incident to billing under the physician’s provider numbers to occur to compliantly collect for services.

What surprise changes did CMS/Medicare make for 2026 for the supervision of NPPs?
