Question regarding billing for Assisted Living Facility. Is the correct POS = 13? Are the CPT 99 codes for E&M Initial visits 99324-99328? Are the CPT 99 codes for E&M Subsequent visits 99334-99337? The same facility provided Independent living Apartments. Is POS = 12? Are the CPT 99 codes for E&M Intitial Visit 99341-99345? Are the CPT 99 codes for E&M Subsequent Visit 99347-99350? Are Routine Foot Care codes 1172_, 11719, G0127, 1105_ with appropriate Q_ and X_ modifiers used as In Office and SNF with the use of POS 12 & 13? Thank you.
