Correct Coding for Telehealth-Patient in Hospital Prior to Follow-up Appointment for Ulcer

I did a telehealth on my patient but unfortunately few days ago prior to my telehealth to check on her ulcer, she ended up in hospital for stomach pain. I was in my office,pt was in hospital . i spoke to her for 15 minutes. so is that 99213 95 or 99232 95 modifier? and if its 99232 on the box 32 of 1500 form do i put my office address or hospital name and address that pt is in now? thank you

The code that would be billed depends on how the patient and the provider communicated about the ulcer follow up. If the conversation was just over the phone with no video capability, the encounter would be billed with a 99442 with modifier -95 and POS 02. If the communication was through asynchronous video (such as Facetime or another messaging application), a provider is eligible to bill CPT 99213 with modifier -95 and POS 02. Billing CPT 99232 would only be appropriate to bill for telemedicine services if it was related to the treatment of the patients hospital care.
