How to Obtain Prior Auth for CPT 97597-Fidelis Care NY

Code 97597 is not covered by Fidelis Care NY. How to obtain a prior authorization for this procedure?

Authorization can be requested by Fidelis Care for wound debridements in two ways:

• On the Fidelis provider portal, requests can be sent electronically for any procedure needing prior approval (https://providers.fideliscare.org/Login)
• Alternatively, requests can be faxed to Fidelis Care with a prior notification form with all supporting documentation (https://www.fideliscare.org/Portals/0/Providers/FormsApplications/2019-FidelisCare-PriorAuthorizationRequestForm-English.pdf)

The process is the same regardless of the type of plan (Medicaid, Medicare, or metal/exchange plans).
