Correct Billing for Removal of 4th RT Nail-No Anesthesia-Dressed with Coban-Paraplegic with Foot Numbness

I saw a patient with a loose nail 4th right which I removed with no anesthesia;he is paraplegic with numbness to his foot.He bled somewhat.I dressed with coban and prescribed antibiotics.How is this billed?
