SSA-721 FORM NOT REQUIRED BY SSA FOR OHIO DEATHS

Due to the success of EDRS, the Social Security Administration (SSA) no longer requires an SSA-721 form to report an Ohio death. The SSA is satisfied with the daily electronic data transmission from the Ohio Department of Health to SSA. While SSA no longer requires the form for Ohio deaths, funeral directors may still issue the form along with its explanatory documentation to the surviving family to assist in settling affairs.
