New Advance Directive Forms

The OOA is a member of the Ohio End of Life Collaborative. Please be aware that, effective 12-16-04, all new Advance Directives signed by patients in Ohio will need to be executed on the newly revised forms (www.ooanet.org/pdf/livingwill.pdf). The new forms were developed with the assistance of the Ohio State Bar Association and were required as a result of legislation passed earlier this year.

The Ohio Hospice Organization has printed the fourth edition of "Choices: Living Well at the End of Life", which is widely used by hospitals and physicians to provide standard forms needed for Living Will, Healthcare Power of Attorney, DNR and Organ Donation. Previous editions of this booklet, including the one that was distributed by the OOA, should be destroyed along with any old standardized Advance Directive Forms or booklets you may provide to patients.
