Ohio BWC Shares How to Complete an AC-2

The agency is getting a deluge of requests right now

With the close of the July 1, 2023, policy year and submission of the 2023 true-up reports underway, the Ohio Bureau of Workers' Compensation (BWC) has experienced an increase in Request to Add/Change or Terminate Permanent Authorization (AC-2) form submissions for processing. Due to this, the agency is reminding employers that AC-2 forms completed and signed by them can be entered online.  

"Completing the AC-2 online will provide you quicker access than emailing or faxing the completed AC-2 to [the] BWC," the agency stated in an alert to employers.

According to the BWC, here are the instructions to update an AC-2 online:

  1. Log-in to the BWC website using your OHID credentials.
  2. Select the online AC-2 form found on Employer Forms webpage.
  3. Click on the green "Start" button.
  4. Enter the policy number and then click "NEXT."
  5. Confirm the information on the screen matches what is on the AC-2. If the contact information does not match, please contact the employer to correct the issue and exit the online AC-2. If the information matches what is on the AC-2, click "NEXT."
  6. Click on the appropriate circle to add or terminate a representative.
  7. Check the box – I have in my possession a hard copy of the AC-2 form signed by the employer – Click "NEXT."
  8. Enter your representative ID number or name and hit SEARCH. (If you do not have a representative ID number, you can request one by submitting the R-4 form.)
  9. Click on the circle that lists your representative ID number and name and click "NEXT."
  10. Select the type of authorized Representation from the drop-down menu and click "NEXT."
  11. Enter name, title of person who signed AC-2; Enter the actual date AC-2 was signed.
  12. Review all the information you have entered to ensure it is correct. If you need to make corrections, click the pencil/paper icon next to the section header where you need to make changes. If the information is correct, check the box: "I have read and agree to the terms and conditions." Enter your initials and then click "Submit."

According to the BWC, employers should have access to the policy within 24 hours of entering the AC-2 online. If an employer completes on online AC-2, the employer is required to maintain the original signed form for the duration it represents the employer; employers must be able to provide a copy of the original signed AC-2 to BWC if requested.
