DEPARTMENT: Nursing Division
LOCATION: 416 S. East Street, Lebanon, Ohio
HOURS: Full-Time (37.5 hours per week), M-F, 7:30-4:00 pm OR
Optional 4-day work week: 7:30-6 pm (3 days) & 7:30 – 5:30 pm (1 day)
PAY RANGE: Pay Grade 7 starting rate of $28.25 hourly, includes full WCHD benefits package
POSTING PERIOD: This notice is being posted for a period not less than seven (7)
consecutive calendar days beginning 02/11/2025 and until position if filled.
MINIMUM QUALIFICATIONS:
Bachelor’s degree in Health Education, Social Work or a related field and one year relevant experience and/or training.
Knowledge, Skills and Abilities:
. Knowledge of community health education, assessing community health needs and program
implementation; psycho-social risk assessments for all clinic clients; knowledge of health education;
. knowledge of risk factors/areas of concern; knowledge of safety and injury prevention; and
methodologies in prevention and treatment of disease;
. office practices and procedures; basic computer skills.
Job Responsibilities: Includes but not limited to:
. Provide safety, injury prevention, and health education to agency and community members; assist with mobile health program;
. Develop and implement health education and prevention programs, assess community health needs;
. Provide psycho-social risk assessments and case management to clinic patients; documents risk factors/areas of concern; make necessary referrals and provide community resources.
. Help manage assistance programs such as Car Seat Program and Infant Safe Sleep Program.
. Provide case management for all SIDS cases in the County; work with the Child Fatality Review Board.
. Serve on various consortiums and other community projects; participate in Emergency Preparedness and Accreditation.
