Health Educator (HUB Care Coordinator)

Union County Health Department

Intermittent, Unclassified, Hourly, Non-Exempt. Monday – Friday, with a minimum of 6.00 hours per quarter and a maximum of 1,000 hours per fiscal year.

The pay range for this position is $17.44 - $23.58 per hour, based on experience .

Serve as the HUB Project Coordinator for Union County. Plan and develop a Pathways HUB or similar care coordination model; design the composition of the program and any public health promotion; coordinate and evaluate programs to ensure compliance with standards; recommend changes to services, materials, and/or operations; create training curriculums for community health workers; and attend coalition and planning meetings throughout the community. Manage the day to day activities and services of the HUB. Assist with person-centered HUB services; serve as the point of contact for client information, referrals, etc.; create monthly reports; assure data entry of client documentation forms; and act as the lead for agency community health workers providing HUB services. Research effective, evidence-based projects. Write and assesses grants, manage multiple grants, conduct financial and programmatic tracking and reporting, communicate with funding sources, complete documentation, and oversee/monitor all related projects/activities. Research new funding opportunities. Serve as a resource to the community and other organizations/agencies. Participate on or convene committees, work events, and special projects. Prepare reports, correspondence, newsletters, media, and other educational materials for the public. Answer general public health-related inquiries and maintain regular contact with partners.

Must be detail oriented, organized, possess strong oral and written communication skills, excellent time management skills, and strong customer service skills. A Bachelor’s degree in Public Health, Nursing, Education, Science, or a related health field; or an equivalent combination of education, training, and/or experience is required. Experience with Microsoft Office is essential. A valid Ohio Driver’s License, valid auto liability insurance, the ability to maintain auto liability insurance, and the ability to maintain insurability under the County’s vehicle insurance policy necessary. Must be able to occasionally lift, push, and carry objects/equipment weighing up to 25 pounds unassisted and up to 50 pounds with assistance   Experience working with GMIS 2.0 or other grant platforms beneficial.
