Updated MasterChoice Claim Form

Just a reminder to use the updated MasterChoice Form!

MasterChoice has updated the claim form to include a new section to complete if the funeral home being paid is different than what is listed on the death certificate.

Please use this form going forward and you may copy it onto your letterhead.
