Payment Innovation in Affordable Housing: May 17

LeadingAge Ohio is calling on Housing Leadership, Bankers, Developers, HCBS Providers, Health Plan Leadership and Policymakers to join us for a day of mapping solutions for funding community-based aging services. The Ohio Departments of Aging, Medicaid, as well as several payors have already confirmed their attendance- make sure to register soon to ensure you are at the table!

Ohio’s aging population will reach its peak by 2035, while its working-age population shrinks. Communities will need to optimize existing, low-cost care settings with just-enough services that maximize older adults’ wellness and function, staving off reliance on health systems.

The day will begin with a high-level view by Robyn Stone, Director of the LeadingAge LTSS Center, on where aging services are headed and what new practices offer the most promise. On-site service coordination is one solution, and participants will learn how three different partnerships have worked—or not worked—to create sustainable funding streams for this vital service. Later in the day, participants will hear about different models that allow healthcare providers to assume full risk for the population they serve—to put the control in providers’ hands. The day will conclude with table work, with participants rolling up their sleeves to chart next steps.
