The day will begin with a high-level view of where aging services is headed and what new practices offer the most promise. On-site service coordination is one solution, and participants will learn how different partnerships have worked—or not worked—to create sustainable funding streams for this vital service. Later in the day, participants will hear about different models that allow healthcare providers to assume full risk for the population they serve— at a significant savings to payors.
