The Center for Medicare and Medicaid Innovation is interested in learning more about the experience of senior living organizations that either lead or engaged with ACO REACH entities as provider partners to learn what aspects of the model supported coordination between these two groups and areas where further work could be done to improve opportunities for partnerships between seniors living organizations and ACO REACH entities.
