The FX modifier is only required to be used if the physician uses an X-ray machine that produces results on film versus a digital image when performing a radiology procedure. If the machine uses film, then all radiology services must be appended with the modifier, which should be billed to all insurance carriers, not only Medicare and Medicare Advantage plans.
