MCR Requirements When Filling Out HCFA-Referring, Ordering & Supervising Providers

It seems that Medicare requires us to use Supervising provider with the DQ in Box 17 of the HCFA instead of referring physician for covered hazardous foot care. When did this change?

Per the Medicare LCD for Routine Foot Care, there have been some recent changes, but nothing regarding referring, ordering, or supervising providers. Claims should be billed to Medicare as they always have been, with the following information depending on the systemic condition reported on the claim for Routine Foot Care:
