Reinstatement or Reactivation of Medicaid Enrollment

hi Hoda- We are having an issue with our medicaid registration. Apparently they sent a request for information to an old email address. After we didnt respond, they suspended me. We have been trying to fix this but its been very difficult. they dont accept email or faxes. Everything goes snail mail. Nobody answers any phones and voice mail messages dont get returned. Can you help us. We have not been able to bill for over a year. 

Per eMedNY website, the Enrollment form needs to be completed for the following 3 reasons:

Complete this Enrollment Form if you are: 

  1. Applying for initial ENROLLMENT or ALREADY ENROLLED and enrolling another NPI, or
  2. Responding to a letter instructing you to REVALIDATE your enrollment, or
  3. Seeking REINSTATEMENT or REACTIVATION of your previous enrollment

Since your application was submitted 2 months ago, the completed application will be reviewed by the NYS Department of Health. The determination will be sent to you, in writing, upon approval or denial of the application. If more information is necessary to process the application, you will be notified as to what information is needed and where to send it. If the application is approved , you will be notified of your provider ID Number and the effective date when services may be provided to an enrolled client.
