Correct Billing for Casting Materials for Custom Orthotics

Can we bill and get payment from the insurance companies for casting materials for custom orthotic castings? If so what billing codes do we use? I know we can not bill out the custom orthotics until dispensed.

CPT Code		Description
A4580	Cast supplies (eg plaster)
A4590	Special casting material (eg fiberglass)
29799	Unlisted procedure, casting or strapping
S0395	Impression casting of a foot performed by a practitioner other than the manufacturer of the orthotic

You may bill for the application and the actual casting material, however, not all insurance companies will pay.
