Proper Billing for L4361-Commercial Insurances

L4361 was submitted to Fidelis with no modifier and denied for missing a modifier. Is KX modifier used for Medicare and commercial insurance companies? And is RT and/or LT always necessary??

When submitting L4361 to commercial insurances, please note that RT and/or LT modifiers are always required.The KX modifier is used when submitting to Medicare and some Medicare Advantage Plans.
