Proper Timeframe for Established Patients to Fill Out New Intake Forms

When seeing a patient after 1.5 years we have to have the patient fill out a whole new intake forms as if its a new patient? How long after a patients visit does the patient has to fill out completely new intake forms ?

There is no set time frame on when new patient forms should be filled out by established patients. It is however important to ensure that information is accurate by asking each time they present to the practice, if anything has changed with their demographics, insurance, and pertinent medical information. This is an aspect that is truly under the discretion of the practice.
