Correct Billing for I&D of LT Foot with Cellulitis and Deep Abscess of Strep Infection-Bone Level-Toe and MPJ Area (2 Separate Incisions) w/o Bone or Soft Tissue Resection

I have done Incision and drainage of Lt foot cellulitis and deep abscess of strep infection with probing the area deep to the level of bone around the toe and MPJ area (with 2 separate incisions) w/o bone or soft tissue resection; what would be the correct cpt? Dx: acute OM, NIDDM , ulcer

The CPT code that should be billed for the surgical procedure is 28003 (incision and drainage, below fascia, with or without tendon sheath involvement, multiple areas. ). When the procedure is billed to insurance, it should be billed with a site modifier that indicates what foot was operated on. When appending diagnosis codes for the surgery, the OM diagnosis should be primary with the ulcer diagnosis secondary.
