Program Objectives

Upon completion of this workshop, the learner will be able to identify common structural and social determinants of health in US populations and their relevance in patient care. The learner will be equipped to recognize how structural and social determinants of health intersect to affect patient outcomes. The learner will be able to integrate new skills and tools to address structural and social determinants of health in their clinical practice.

By the end of the presentation, attendees will be able to: Compare health outcomes and community characteristics across Montgomery County; Utilize tools containing health data to facilitate health programming and patient care; Identify community resources to support patients expressing social needs.

Upon completion, the learner will be able to document, code, and bill for encounters relating to Social Determinants of Health.
