CIGNA Government Services mailed letters to all Ohio Medicare providers Ohio, March 12, alerting them that they need to complete and file a revised 588 Authorization form or they will not receive payments when CIGNA takes over for Palmetto GBA.
Providers who currently bill National Government Services (NGS) and are currently enrolled with NGS must submit a revised CMS 588 Authorization Agreement form to CIGNA Government Services immediately to receive payments electronically.
It is imperative that each provider completes an EFT Authorization Agreement and returns it to CIGNA Government Services no later than May 18, 2011.The Electronic Funds Transfer Authorization Agreement is located on the CMS website at: http://www.cms.hhs.gov/cmsforms/downloads/CMS588.pdf. CIGNA Government Services was selected as the Medicare Administrative Contractor (MAC) for Jurisdiction 15 A/B MAC for the states of Kentucky and Ohio. The transition to CIGNA for physicians begins in June.
