About the Opportunity:
The Ambulatory Care Pharmacist is responsible for providing comprehensive medication management (CMM) services to medically underserved and vulnerable populations within a Federally Qualified Health Center (FQHC). The pharmacist works as an integrated member of the Patient-Centered Medical Home (PCMH) team to improve clinical outcomes, medication access, and health equity. The pharmacist collaborates with medical providers, behavioral health, case management, and enabling services staff to optimize pharmacotherapy, address social determinates of health, and improve HRSA Uniform Data System (UDS) quality measures. Services are delivered under collaborative practice agreements and include chronic disease management, medication reconciliation, patient education, adherence support, and transitions of care. Provide exceptional customer service to patients and staff. Provide face-to-face and telehealth medication management visits. Initiate, adjust, or discontinue medications under collaborative practice agreements Develop individualized care plans based on patient goals and barriers. Conduct medication reconciliation and polypharmacy review. Order and interpret labs per protocol. Provide culturally competent counseling using teach-back methods. Document visits in the EHR consistent with FQHC standards.
