Residents are provided the opportunity to accelerate their clinical skills beyond entry-level professional competencies in patient-centered care and disease state management through a shared visit model and collaborative practice agreement. Upon completion of the program, the resident will be able to provide medication therapy management (MTM) to patients, provide services designed to promote community wellness, provide disease state management, assist and potentially lead clinical program development as well as a dispensing role and optional teaching certificate. Family Health Services is an FQHC that utilizes team-based patient care, project management, and a diverse patient population.

University Hospitals’ Postgraduate Year 1 (PGY1) Community-Based Pharmacy Residency is a full time, 12 month program that provides exposure to a variety of patient care experiences within community and ambulatory practice, as well as providing experiential and educational opportunities to enhance knowledge base and promote personal growth. Residents will have opportunities to develop their clinical skills through patient care services, such as medication therapy management and transitions of care, as well as obtain a teaching certificate and precept pharmacy students.
