ODI survey to identify problems with provider network adequacy

The Ohio Department of Insurance is asking for feedback about provider network adequacy. This applies to Marketplace and non- Marketplace health plans. Individuals are being asked to complete a brief web-based survey to provide consumer feedback into a rule that will be finalized soon. 

You may submit this survey anonymously. However, if you do use your name and contact information to request follow-up, the survey response will be treated as a complaint and therefore is considered a public record except for any specific medical or other confidential information that is provided along with the complaint.
