ODH Patient Centered Medical Home Will Begin Soon

The Ohio Department of Health is pleased to announce that primary care practice transformation, made possible through a two-year PCMH pilot project established by House Bill 198, will begin soon! Through funding from the Ohio Department of Job & Family Services, the Ohio Department of Health has been able to hire a vendor to train 50 participating practices to transform into Patient-Centered Medical Homes.

A Request for Proposals (RFP) was released to identify the vendor who can best facilitate the training and implementation process of practice transformation in Ohio; those proposals have been reviewed and scored, and we’re happy to report that the contract has been awarded to TransforMED. We will work through details and timelines with TransforMED and soon be able to provide more information about the two-year pilot project.
