Community Health Worker

Union County Health Department

Full-Time, Classified, Hourly, Non-Exempt. 37.50 hours per week, Monday – Friday, with regular
evening and weekend community events. The pay range for this position is $16.50 – $23.10 per
hour, based on experience. Funding for this position is provided by the Regional Prevention and
Linkages to Care Collaborative grant.

Partner with other community agencies to help at-risk / high-risk individuals and their families
navigate complex social service and health care systems to services that promote healthy
behaviors and manage conditions that affect their health and social well-being. Facilitate client
access to community resources and assist clients in utilizing community services. Document client
encounters and contacts made on behalf of clients. Complete and submit monthly reports.
Document activities, service plans, and outcomes; and maintain electronic client files. Facilitate
communication and coordinate and monitor services between providers and clients. Build and
maintain positive working relationships with the clients and their health care providers; and assist
with education and support to improve health care outcomes. Conduct general outreach within
the Central Ohio subregion designated for the project; including traveling to sites across the
region to provide technical assistance and care coordination. Assist with the documentation
required to maintain grant funding, and with the development of standard operating guides
(SOGs) for the program deliverables. Attend required training and meetings. Assist the Drug
Overdose team in general planning, implementation, and evaluation of the program strategies and
deliverables. Serve as a community resource, regularly assisting with and representing the agency
at general community events and activities. Serve as a car seat technician.

Must be organized, possess strong oral and written communication skills, excellent time
management skills, and strong customer service skills. A strong attention to detail, ability to
perform data entry with a high level of accuracy, ability to multitask in a busy setting, and ability to
work effectively with a diverse range of clients with various health and social service needs are
essential. High School Diploma or equivalent; previous experience answering phones and greeting
and assisting clients/visitors; and proficiency using computers and computer software (including
Microsoft Office) required. Must currently possess, or obtain within one year of hire, certification
as a Community Health Worker; and must currently be, or be willing to become, a certified car
seat technician. Ability to maintain required certifications essential. Ability to lift, carry, move, or
push files and/or packages up to 25 pounds unassisted and up to 50 pounds with assistance
necessary. Experience as a Peer/Patient Navigator, Peer Support Specialist, or Case Manager;
knowledge of Substance Use Disorder (SUD) best practices or other health disorders; and
experience working in health care including behavioral health, public sector, or public health
desired. Previous experience in a secretarial position; performing patient assessments;
performing care coordination; with an Electronic Medical Records (EMR) system; and a valid Ohio
Driver’s License, valid auto liability insurance, the ability to maintain auto liability insurance, and
the ability to maintain insurability under the County’s vehicle insurance policy are required.
