We Answered: CMS has posted the information for the Risked Based Surveys on the CMS Nursing Home webpage under the download section. The Survey Resources zip file contains:

• RBS CMS-802
• RBS Entrance Conference Form
• RBS Facility Tasks Job Aid
• RBS Mapping Document Streamlined
• RBS Procedure Guide
• RBS Screening Job Aid

Facilities are encouraged to become familiar with these resources before implementation of the Risk-Based Survey process. In particular, reviewing the Entrance Conference Form, Screening Job Aid, Facility Tasks Job Aid, and applicable survey pathways can help facility teams understand the information surveyors will request and the processes they will use during the survey.
