2025 Conference Evaluation Form

2025 MHOA Annual Conference: Session Evaluations

Please complete an evaluation for each of the sessions you have attended.

All sessions qualify for CHO, CPH, and RS/REHS contact hours. All wastewater track sessions and plenary sessions qualify for SE/SI training contact hours (TCH). Food sessions qualify for Certified Professional – Food Safety  (CP-FS) contact hours. Approved nursing CEU sessions are highlighted in pink on the conference program schedule.

All conference attendees are responsible for tracking their continuing education contact hours and submitting any required documents to their respective credentialing agencies.

Please note: Your contact hours may not appear in your MHOA profile until after Thanksgiving. Please email questions after Monday, December 1 to info@mhoa.com.
