This program is a must for all pharmacists interested in getting paid for disease state management. Pharmacists are in a great position to provide pharmaceutical care services for diabetics and help patients reach clinical targets and improve their outcomes.

Learn how you can have hands-on management of patients’ behaviors and increase their compliance with medications. This is a tremendous opportunity for pharmacists to expand their role on the healthcare team, while at the same time significantly increasing revenues.

This program is funded by a medical education grant from Ross Products Division.

GOALS AND OBJECTIVES
Diabetes Collaborative Practice Between
Pharmacists and Physicians in a Family Medicine Office Setting

At the conclusion of this session, participants will be able to:
1. develop a collaborative on-site practice by pharmacists and physicians within a family practice of medicine, to manage diabetic
patients;
2. describe the clinical services provided by a pharmacist in a collaborative physician /pharmacist practice for the management of diabetic patients;
3. review outcome measurements to substantiate the importance of the collaborative practice management of diabetic patients by pharmacists and physicians;
4. identify a business plan for both the physician and pharmacist on how the collaborative practice between both physicians and pharmacists would occur; and
5. identify payment sources for clinical pharmacy services.

At the conclusion of this session, participants will be able to:
1. identify nutritional interventions important to diabetes, COPD, and osteoporosis;
2. describe why these interventions are important in chronic disease states; and
3. demonstrate an understanding of the role of the pharmacist in educating patients with these disease states about the importance of nutrition.
