The information may be printed and used as a bag stuffer, as a display for your waiting area, or as part of an ad. This resource will be updated approximately every six weeks. Patient Counseling Survey
My name is David Tietz and I am a P5 at Ohio Northern University (ONU). I am conducting a survey as a part of my ONU Honors Program Senior Honors Project. The goal of the survey is to analyze trends in pharmacists' counseling of patients across Ohio. The link to access this survey is being sent to community pharmacists who are members of OPA. If you do not practice in a community setting where you dispense medications, please disregard this survey.
I would appreciate a few moments of your time to complete this short survey, which can be accessed using the link provided below. I anticipate it will take less than five minutes. Please be as honest and complete as possible in completing the survey. I hope to use the results from this survey for a poster presentation, and to educate pharmacy students on improving counseling practices.
