Free OSHA Audits Available

RE:                  OSHA Audits

As you may be aware, the Ohio Bureau of Workers' Compensation requires OFDA to meet various standards annually in order to comply with the Group Rating Safety Program. 

As a part of this compliance, OFDA will offer complimentary safety audits to those funeral homes that participate in the OFDA Group Rating Program.  OFDA has contracted with Barb Garrison of Environmental Programs to provide these confidential safety audits. 

If your funeral home would like to participate in a complimentary audit, please fill out the form below and return it to the OFDA office.  We will randomly select twenty member funeral homes to receive the safety audits.  You will receive a letter if your funeral home has been chosen.

Please be assured your audit will be absolutely confidential and only the acknowledgment of the completion of the audits will be communicated to the Bureau of Workers' Compensation.
