Dues Schedule
Dues are calculated by a base fee, plus the number of death certificates filed for all locations of the firm during the previous calendar year. The price per death certificate is categorized in tiers below. Death certificate amounts go through each applicable tier at that rate.
EXAMPLE: 650 death certificates:
- Base Fee (one base fee per company, not per branch location) = $220
- 500 death certificates at Tier 1 (x $3.50) = $1,750
- 150 death certificates at Tier 2 (x $1.75) = $262.50
- Total Due = $2,232.50
Upon approval of your membership application you will receive an invoice to pay your dues based on a prorated amount depending on the time of year your application is approved.
Annually, OFDA members are mailed dues invoices in November. Deadline for remittance is February 28th.
